Our catalogers report that there is a problem with Koha's duplicate-checking in
addbiblio.pl. According to their report, when a duplicate record is suspected,
Koha displays an error message (correctly), and offers a link to the suspected
duplicate:

Is this a duplicate of [link to existing record title]?

You must either :

    * If it is a duplicate, Edit Items of the existing record.
    * If not, click to Confirm it's not a duplicate

>From this screen the catalogers click the link to the existing record in order
to edit that record and add a new item. However, they report that the record
that they originally entered (which was found to be a duplicate) is still
added. This results in many orphaned duplicate records with no items attached.
