When a patron suggests a purchase you want fields like title, ISBN and maybe
even URL where they can link to you the title in question.  Right now Summary
is confusing - and notes means that the librarian has to do a lot of reading
(potentially) to find necessary information.  I've filled in suggestion forms
before and they were always more robust - you want to give the patron a lot of
fields to try and fill in so that the librarian doesn't have to do too much
guessing or searching.
