Proposed fix

- Moving nested TMPL_IFs out of <option> tags (breaks translator)
- Fixing markup to match conventions for form structure
- Removing duplicate form fields (bad merge?)

FIXME: Template contains LOTS of TMPL_IF EXPR (See Bug 2670)
