I was actually thinking a new table using only patron category and branch would
be the ideal solution. I was hoping to fix it without removing holdsallowed
from the issuingrules table, but I think that approach is just untenable at
this point. I should be able to create the new table/columns and import the
issuingrules.holdsallowed where possible.
