Bug 10662: (QA follow-up) addressing qa test tool output

I've done the following in this follow-up:

- Added missing POD
- Replaced DBIC use with Koha::Object[s]
- Replaced DataTables with KohaTable
- Fixed database charset and collation
- Moved Javascript to bottom of templates
- Fixed a syntax error in rewrite-config.PL
