I like this idea generally, but I'd prefer a method to define fields as
desired, rather than a set number of fields -- something more like a patron
attribute rather than something like sort1/sort2.

In fact, could we make this an option on patron attributes? A checkbox in
attribute setup for something like "Show in housebound." If that's checked,
this attribute shows in the housebound module rather than in the usual place?
