Actually, I think that you just unlocked this problem in my mind. Looking at
bug 14695 it looks like it's limited to only multiple bib-level holds? Bizarre. 

It looks like Bug 15565 will fix this. 

I'm keen to get Bug 15565 pushed, but I think Victor pointed out an issue with
the hold count logic, so just waiting to hear from Aleisha.

I'll update the bug name there and mark this as a duplicate.
