Bug 29323: Add modal dialog and content divs to overdrive checkout modal

To test:
1 - Enable overdrive integration and circulation via Koha sysprefs
2 - Search the opac - view overdrive results - checkout an item
3 - You should now have a 'Download as' button
4 - Click it - it doesn't look right
5 - Apply patch
6 - Click the button
7 - Modal appears correctly
