It's not really unique to the US. It's common to "English" countries like the
US, Canada, Australia, New Zealand, and the UK. 

That said, I don't often see fields in Australia for middle name. Typically I
see either "First name" and "Last name", or "Given names" and "Last name".
Occasionally I'll see a field for middle name. 

I actually have found previously that giving my middle name in situations has
led to data errors. For some reason I end up being referred to by my middle
name instead of my first name (both in Canada and Australia), so I try to omit
it wherever possible.
