Hi Michaeal,

could you take a look at changing the form as suggested in comment#19?

The patron edit form could be a good starting place for this.

I realize there is another place in Koha at least where it's done like you do
here - the statistics wizard for patrons - but I believe it would be worth
changing and changing it there too.
I can easily see a library using the field for individual notes.
