I've added a hold_cancellation_requests table to store all we might need about
the cancellation request. Patron's will be allowed per-circ rules to request
cancellation and visual indicators will let librarians know if there are
waiting holds requiring attention, haven't got there yet. It will be API-driven
as much as possible.
