I can't replicate the issue on master nor 22.11.03. I filled out all the
required fields and it saves correctly.

I am setting this WORKSFORME. If you need to reopen, please try to provide more
information: 
* What is your exact Koha version?
* What do you do to trigger the error? (which values have you entered into the
form?)
* Take a look at the server side logs, the 500 should be explained more there.
