I think maybe it will be easier to keep it simple and have a text field or a
pull down from AV depending on config first. As Kyle said sort fields can have
unique values, so you will have an endless pull down with one entry per patron,
this is not useful. 

Also: pull down and free text will match how this field is used in all other
places in Koha (= existing pattern).

Further improvements could then be made on a follow-up bug.
