I was able to reproduce this bug exactly on a 21.11.11 instance following the
steps set out by Minna Kivinen. Before I followed the steps I created a brand
new item and placed a hold on the title it was attached to; it correctly
appeared in the Holds to pull. After following Minna's steps the title was not
appearing in the Holds to pull, even though an item was available at its
homebranch. It was only appearing in the Holds queue.
