Hi, this is most likely a configuration error. Some things to check: 

* does it happen for all your items or only some?
* Is there a hold on the item and is the HOLD notice formatted correctly?
* Does the item have item type, holding branch and homebranch set?

Also you should check your server logs to find the more useful error.

This classifies as a support request rather than a bug/enh, please report your
finding to the mailing lists or IRC chat if not resolved by any of the above.
