Bug 29324: Add missing copyright and license info

This patch adds a new perl file to prepend
copyright and/or license statements to all files
that are missing them in the directory
(and subdirectories) in which it is run.

To test:
1. Apply the patch
2. Run perl xt/find-license-problems.t
3. Note there are some fails
4. Run perl xt/prependcopyrightandlicense.pl
5. Run perl xt/find-license-problems.t again
6. No more fails :)
