In the staff client, these 4 addresses appear in the same area next to a label of "Physical address". The addresses appear to belong to one field here, and are not separated by a space so the data looks bad. Clicking on Edit allows you to edit the "Physical address" box (containing the combined data from these 4 fields). If you modify the text in this box, and click save, you've modified the location of the data in the database.

From that point forward, the vendor only appears to have data in address1, and address2-address4 are set to NULL.
