This causes a couple of different symptoms depending on system settings, the more common of which are : (1) the select box for 'next available item' does not appear. (2) the place hold button does not appear. This occurs when no bib-level itemtype is defined. As Darrell points out, some checks may then be bypassed, and itemtype icons will not properly display.
