A popular item with many holds gets put back on the regular shelf because it failed to indicate it was on hold. The patron forgot to check it out and it got returned to a different branch. I reported a bug similar to this in 2012 when we were first on Koha. The problem was fixed, but now it's back. I think the original bug was about the hold losing focus.
