I will attach a patch for the staff interface (memberentry.pl) that could be a starting point for this idea. It simply extracts the address blocks to 3 includes (see patch that follows). The behaviour while adding / editing a patron will be the same as before. As a next step, different includes could be added for the different country styles, along with the functionality to select them using the syspref.
