We just ran into this again, giving it a little nudge :)

In Germany I think it would be:

<street name> <street number> <zipcode> <city>

We don't use <street type> at the moment, I think often the part that would be street type is already part of the street name (-gasse, -straße).

The state is not needed for addresses within Germany.
