A patron is not blocked on renewal if an item cannot be held, and an item that cannot be filled by policy does not trigger at checkin. I tested with various settings and was unable to recreate. The tables mentioned here also no longer exist as everything has moved to the circulation_rules table. There still remains an issue of zero holds allowed versus 'no holds allowed' - bug 37792 has been filed as an enhancement there.
