Yesterday we noticed the sysadmin made a typo on the file and it was causing the init file to be skipped silently on startup. I wrote a small script to avoid human errors 

I suggest you try that SQL file right now and see if it loads.

I can share that simple script.

We have it under Xenial and no problem anymore with autoincrements.
