Thanks to everyone that pointed me at various things. It’s helped point me in a workable direction, I believe.
 
However, what would be even more useful are getting sign offs for some bugs:
Bug 14533 - koha-create --use-db option shouldn't create any db or db user

These three bugs would let me set environment variables to build a temporary password file to then setup access to a pre-existing remote database.
 
Granted, there might still be problems:

Which tangentially touches on:
Bug 17237 - Stop koha-create from creating MySQL users without host restriction

Though, there is nothing to sign off yet for this one.
 
I’d prefer to not custom roll a distribution, but rather seek community assistance in pushing these bugs through. However due to time constraints, I probably will roll one, even if it is probably not the best way to roll one (https://wiki.koha-community.org/wiki/Building_Debian_Packages_-_The_Easy_Way).
