OK, based on even more experimentation, the problem is clearly isolated to
Net::Z3950 - using the yaz-client proogram to talk to z3950.loc.gov works fine,
and as I stated above, the Net::Z3950 test program doesn't work, so it's clearly
not a Koha issue - I'm closing this bug. I'm subscribing to the Net::Z3950
mailing list in the hopes of finding a solution...
