MySQL is catching duplicate barcode entries and generating lines to the error log:

DBD::mysql::st execute failed: Duplicate entry '33000000057152' for key 2 at
/usr/local/koha/intranet/modules/C4/Biblio.pm line 1671.

However, the addbiblio routine that is supposed to warn staff about duplicates
is not working.
