Yes, categoryitem definitely looks to be the most flexible.  It might be
difficult, however, to load.  Seems like you'd need a row for every itemtype for
a given categorycode if you wanted to give the categorycode extended loans for
all itemtypes.

Note that this enhancement might also address bug 289.
