All the production Koha's I know about are running at least MySQL 4 if not 4.1.
3.23 has major speed limitations (no query_cache) so any sites not using 4
should probably upgrade. Since 4.x is the default for most modern distros these
days (yes, even Debian ;-)), we should at least warn if our default password
type isn't set correctly for that version.
