It's quite simple:

1. create a brand new serial
2. receive the first issue
3. receive the second issue
4. receive the third issue
5. receive the fourth issue
6. now look at the 'received issues' it will look like:

,Vol 1 No 1,Vol 1 No 1,Vol 1 No 1,Vol 1 No 2,Vol 1 No 2,Vol 1 No 1,Vol 1 No
3,Vol 1 No 3,Vol 1 No 2,Vol 1 No 1,Vol 1 No 4

quite a serious bug!
