Update: the item is added to the record, except for manual issues where it is
not. Koha does not currently check for duplicate barcodes in serials
itemization, correct HDL?

This is a known bug, filed as 1144, HDL is working on a fix I believe.

Not currently a field used in serials itemization

This is described in bug 1125. We will find a solution asap.

This sounds like perhaps a bug in the old serials module, and might not be
related to the upgrade. I will investigate and attempt to get more information
from the client.
