It seems that any/some timeout causes it to generate an "internal server
error". 
I had a look at koha-error_log which has the following:
  z3950.loc.gov error @attr 1=4 @attr 4=1 "simple": Timeout (10007), referer
... /search.pl?bibid=&title=simple%20faith
  Premature end of script headers: search.pl, referer ...
/search.pl?bibid=&title=simple%20faith
