Overhaul patches sent.  Fixed display of "Add List" input.  Fixed XHTML
validation.  Fixed error display for adding a duplicate list name (could still
use some retouching via CSS).  Added instructional message ("how to add") on
viewing empty list.  Removed obsolete columns for callnumber.  

Note: opac-addbookbybiblionumber.tmpl doesn't exist anymore.

TODO:
 1) update VirtualShelves.t
 2) make pretty like staff shelves
 3) SQL updates: 
~ virtualshelves.owner type=INT (linked to borrowernumber) and probably should
be required
~ virtualshelves.category default=1 (private)
~ virtualshelfcontents.dateadded should autopopulate (Null=NO and
Default=AUTO).
