I've found several problems that are the cause of this:

1. someone removed the template files for marc21 plugins (now patched)
2. someone changed the way plugins work and the previous marc21 plugins needed
a new hidden input with the name of the file to pass as a parameter (now
patched)
3. there is another prob I haven't resolved yet:

When I click on the [...] I now get the error:
