1, 2, and 3 are long-standing problems with 3.0 which haven't been resolved.
We've got a very spotty implementation of call number display and item type
images.

4 is not really a bug in itself, just a symptom of 1 and 2. The check box is
supposed to be associated with an item type image or label. That's why it's
floating there all by itself.
