I'm not sure where these comments belong, with all the different bug reports
relevant to this issue, but here goes: The new "Update Child to Adult Patron"
toolbar menu option is not working for me. I get the javascript confirmation
dialog, but them I'm redirected to a blank page.

I notice the <!-- TMPL_VAR NAME="catcode" --> variable is not being passed to
the template, so the js redirect looks like this:

window.location='/cgi-bin/koha/members/update-child.pl?op=update&borrowernumber=60&catcode=';
