In some OPAC and staff displays, the item type code is displayed, while in
others, the item type description is displayed.  In some cases, both are.

This should be rationalized.  I assume that the OPAC should always display the
description, and only the description, but it is not clear to me when the staff
interface should display the code, the description, or both.

Chris, pending a decision about what changes to make, for now please do an
audit of the code and locate every place (that you can find in two or three
hours) that the item type is displayed and how.  The resulting list should be
added to the bug.
