At check out time if the item being checked out was never checked in, there
either should be:

1. syspref to either charge fine or not charge fine default
2. option on checkout screen to not charge a fine. 
3. both.

If a book accidentally was not checked in and put on the shelf, then the
previous patron is still being fined. Of course, this could be a ploy for a
patron who knows this to put a book back on the shelf to avoid a fine, although
not if the book is never checked out again, or until the patron who tried to
trick the system needs to check more books out. 

This may have something to do with fining patrons upon return rather than
nightly while checked out.
