There are a number of differences in the schema of a 3.0 database that was
upgraded from 2.2.9 to 3.0 vs. one that was installed fresh -- i.e., extra or
missing tables or columns, differences in the type of a column, etc. 

This bug will track changes to kohastructure.sql, updatedatabase.pl, and the
2.2 to 3.0 upgrade to resolve these differences.
