adding items to the list was incorrectly attached to logic for opacbookbag
system preference.  There was also a problem with the way the javascript was
built if the opacbookbag preference was turned off.  Since the javascript
didn't form properly then, the 'select all', 'clear all' and 'add-to' listbox
were not displayed.
