opac-reserve checks for existence of the bib-level itemtype definition.

This causes a couple of different symptoms depending on system settings, the
more common of which are :

(1) the select box for  'next available item' does not appear.

(2) the place hold button does not appear.

This occurs when no bib-level itemtype is defined.

As Darrell points out, some checks may then be bypassed, and
itemtype icons will not properly display.

(note that this script also does not implement the itype/ccode
icon choice feature).
