This occurs on upgrade from 2.2 ; there's no good way to link serialitems
retroactively.  The table structure ends up broken in the OPAC, with a couple
of missing  td's.

This is related to the issue that the serial flag is overloaded to also mean
series.  It's set in the code if there's a series title.
