This is interesting: I started with a lowercase surname. I turned on the
uppercasesurnames preference. I opened my record for editing and my surname
appeared in uppercase. I clicked save without changing anything, and my surname
was now in uppercase (as displayed on the checkout page).

Opening my record for editing again, I deleted my surname and re-typed it.
Again, it appeared in uppercase. After saving, however, it was back to lower
case. In this case there is the /appearance/ of upper case because of the CSS
(text-transform: uppercase), but there doesn't seem to be a mechanism in the
add patron process to actually convert the text.
