When saving a new authority record in authorities.pl, when the admin chooses
"Save" at the bottom of the form, IE7 raises a javascript error: LIne 96, char
13, can't move focus to the control because it is invisible, not enabled or of
a type that does not accept the focus. The new authority record cannot be
saved. 

When using the top "save" button, there's no javascript error but the record
does not save. (The button does not do anything.)

This happens for personal names, topical terms, and the other authority types.
