Following up on a comment Owen made to patch submitted to fix bug 3100, there
are some issues with form inputs to think about:

[1] In general, form inputs should allow data entry up to the width of the
corresponding database column.

[2] It would be nice to find a way to not have to manually update the templates
every time a column width cahnges.

[3] For purposes of visual design, sometimes it's nice to have the form input
(as displayed) be shorter than the maximum width of column.
