Proposed fix

This fix changes the formatting of the guarantor entry fields to make it more
appropriate for professional patron types. It shows only surname to match the
way organizational patrons are stored, and it eliminates the relationship
field. The relationship options might be useful for professional patrons, but
at this time any relationships you define would appear for both child and
professional patrons. I suggest addressing this issue before reintroducing the
relationship field for professional patrons.
