+++ This bug was initially created as a clone of Bug #3118 +++

Two display issues:

[1] captions of some radio buttons and form inputs are compressed horizontally
and stretched out vertically.

[2] Excessive vertical spacing between form inputs.

These labels are all still too narrow:
"create an item record when receiving this serial," 
"do not create an item recodr when receiving this serial", 
"Staff and Public Display", 
"Number of Issues to display to Staff," and 
"Number of issues to display to the public"
