At circulation desk, if a barcode is scanned that is not in the catalog, add an
option to create a "fast add" bib record.  Circ staff can fill in the basics
and check the item out; the item is flagged to be routed to cataloging upon
return for proper record creation.

Feature is controlled by a granular permission (fast_add)which is part of a
separate enhancement.
