There are fields in the serial add item form which I don't think should be
there:

- Koha issues (l)
- Koha renewals (m)
- Koha reserves (n)
- Koha out on loan (q)
- Koha date last seen (r)
- Koha date last borrowed (s)

These are values generated by Koha, so I'm guessing they shouldn't be
user-editable.
