Suppose an item has a holding library of ABEL and an item type of DVD.  If the
hold policy for this branch and item type is set to 'From Home Library', a
patron from branch BAKER should not be able to place an item-level request for
this item, nor should that item fill a title-level request for that patron.

The picklist generated by build_holds_queue.pl was not retrieving the item type
properly, and thus was not retrieving the correct hold policy, with the effect
that the system default of 'From Any Library' was used.
