Add the ability to request a hold on multiple items at once time in the
following contexts:

1. From the search results page.  Add a "Place Hold" button that will apply to
all checked items.

2. From the cart.  Add a "Place Hold" link that will apply to all checked
items.

3. From a list.  Add a "Place Hold" link that will apply to all checked items.

In each case, the user will be redirected to a confirmation page showing the
bibliographic information for each selected item (instead of the single item
shown now).  The user may remove selections before confirming the request.

Additionally, if permitted by the current configuration, the user will also
have the option for selecting a specific copy of each item.
